summary

Modern endoscopis appliances with cold light provide a butter in sight into the bladder, and the aocumulaiting experieinee make possible macrosoopie evałuation of tissues. The authors believe that the new methods enable a more aocurate determination of the degree of bladder wall infiltration by the malignant lesions to be obtained, and more radical removal of the tumour by means of trans-unethral electroresection. This, however, requires. an appropriate anaesthesia. Sińce typical conduetion anaesthesia cannot proof the safety of the patient and ade±uate conditions of the operation to the surgeon the authors envolved a method based on błock of Li?L4 roots at two levels: in the subarachnoid space or epidurally, and at the site of the obturator nerve exit from the smali pelvis. This nerwe is formed by these roots and innemrates the femoral adductor muscles. Only this twio-level oonduetian anaesthesia makes possible a radical operation.
