Urachal abnormalities in the adults are rare. After birth urachus becomes the median umbilical ligament. When the obliteration is incomplete or there is another opening, we can observe following pathologies: urachal sinus, urachal cyst, vesicourachal diverticulum, patent urachus. We reported the case of a 52 years old female patient with an infected urachal cyst, who complained of fever, dysuria and suprapubic pain. Diagnosis was made on the grounds of physical examination and additional studies: ultrasonography, computed tomography, cystourethrography and cystoscopy. Therapeutic management consisted of antibiotics with subsequent surgical removal of urachal cyst with bladder cuff. There were no postoperative complications. Pathologic examination of the specimen confirmed earlier diagnosis. Differential diagnosis of lower abdominal pain with presence of tumor should include infected urachal cyst. Computed tomography is an effective diagnostic tool. Treatment of choice is radical excision of urachal cyst with a bladder dome cuff.
