Anterior urethral injuries are usually iatrogenic and arc most commonly associated with traumatic catheterisation or cystoscopy. Anterior urethral damage may also come from blunt trauma to the perineum or penetrating injury like stab and gunshot wounds. The main symptom of anterior urethral trauma is haematuria, usually associated with penile hacmatoma and urine retention and bladder distention. The treatment (surgical or conservative) is determined by the urethral damage degree.
The data collection: five years data on 255 patients from 63 urological departments in Poland were reviewed. Two hundred and six cases were iatrogenic (catheterisation or cystoscopy associated), 48 patients had peritoneum injury, one urethral damage resulted from straddle injury. The diagnosis was set with retrograde urethrography, voiding cystourethrography and urethroscopy. Conservative treatment was used in 193 individuals, while 62 patients required surgical intervention. The authors concluded early diagnosis and appropriate therapy can result in recovery of urethral function.
