posterior urethra, posterior urethral injury

summary

Introduction. Posterior urethral injuries are usually caused by motor vehicle accidents; may also result from fall from some distance or burying; they can be a part of a multiorgan trauma or isolated injury. However, posterior urethral injury is most commonly associated with pelvic fracture.
Patients and Methods. Survey data on posterior urethral injury registered in 63 urological departments in Poland between 1995 and 1999 were studied.
Results. Two hundred and sixty eight patients with posterior urethral injury were analysed. Most cases were caused by road accidents, isolated trauma or multiorgan (mostly pelvic fractures) associated ones. Diagnosis was set with retrograde urethrography, voiding cystourethrography and urethroscopy. Treatment comprised percutaneously placement of a suprapubic catheter with the perineal or abdominal drainage. In a few cases surgical reconstruction of urethra with the restoration of urethra continuity was necessaiy.
Conclusions. Early diagnosis and appropriate surgical treatment in posterior urethral injuries are essential for regain of urethra function. The placement of a suprapubic catheter with the adequate drainage of the bladder constitutes the least invasive treatment method, which is recommended for less experienced urologists.
