Introduction.

Patient’s prognosis after RP due to prostate cancer depend on: PSA estimated before operation, clinical and pathological stages, grades estimated in Gleason score from prostatic core biopsies and specimens after radical prostatectomy. Gleason score grading remains subjective. Intra and interobserver variability could be found in foreign literature, but not in Polish.

Objectives.

The aim of the study was to evaluate intraobserver variability of Gleason grading of prostate core biopsies and specimens after radical prostatectomy.

Material and methods.

Histological material of patients with prostate cancer was analyzed: 45 after prostate core biopsies and 51 after radical prostatectomy. All of them underwent biopsy or radical prostatectomy between 2001-2007. Gleason score grading were estimated once again by the same, experienced in urological histology and blind to the original score, pathologist.

Results.

In 32 (71.1%) out of 45 core biopsies there was a lack of agreement in Gleason score. In 27 of them first Gleason score estimation was lower than the second. In remaining 5 the opposite. Agreement was reached in 13. In 35 (68.6%) out of 51 specimens there was a lack of agreement. In 28 of them first Gl.s was lower than the second. In remaining 7 the opposite. Agreement was reached in 16.

Conclusions.

This study confirms a significant difference in intraobserver variability both prostatic core biopsies and specimens after radical prostatectomy. Most often it happens that the first estimation is more beneficial than the second (Gl.s1
