summary

Introduction.

Pelvic organ prolapse (POP) is defind as the descent of the Pelvic organs below their normal position, evidenced as a protrusion of the vagina and cervix. The risk factors associated with prolapse are race, anatomy or collagen distribution, vaginal delivery or radical surgery, obesity. The natural history of the development of the POP is still unknown. There is a current theory that indicates that the first step in the alteration of the pelvic floor involves the injury to the levator ani muscles. The major problem with surgery is that it is not always definitive. We do not know how to manage to do it.

Objectives.

The objective of this study is to present the method of Prolift Anterior System, an innovative design coupled with transvaginal mesh technique in ready to use for anterior pelvic floor repair.

Materials and methods.

Film presents operation technique of anterior vaginal prolapse which has been done in our Clinic since 2007. Operated patients had gynaecological examination and according to POPQ classification, stage of prolapse was assessed. In procedure polipropylene mesh (Prolift Johnson & Johnson) is in trapezoid shape and from each angle fixed branch are leaded through upper and lower part of obturator foramens by needle toward perineal skin.

Results.

There were no complications as vessel or urethra and bladder injuries during operation. Two women had fever and required administration of antibiotics. Blood transfusion was never required. The mean hospital stay was 3.25 days (range 3-10). All these patients were examined after 1, 3 and 6 months. No infection or rejection of the polypropylene mesh occurred during follow-up.

Conclusions.

These results suggest that the Prolift Anterior approach is a save and effective method of treatment for vaginal prolapse.
