summary

Issues related to pathophysiology of operative stress associated with extensive urological procedures were described. Metabolic, endocrinological and immunological disturbances occurring early after the operation were discussed.
Reasons of postoperative stress and mediators of stress responses were analyzed. It was emphasied that mechanisms initiating and supporting the stress responses are not all fully identified. The current methods allowing for modification of stress responses were presented.
Short history of development of surgical techniques used in the treatment of bladder cancer and usefulness of different intenstine fragments for bladder substitution were described.
Review of the literature regarding the development of investigations concerning the influence of extensive operations on various aspects of stress responses including current knowledge of disturbances occurring during early catabolic phase after cystectomy with intestine conduit.
