Experimental comparison of wedge and transverse excision of renal lower pole. Morphological investigations

The authors report an experimental work performed for studying the effects of damage to the arterial system of the kidney after wedge resection and trans-verse amputation of the renal lower pole. The materiał included 64 rabbits operated upon with wedge resection or transverse amputation of the lower pole of the left kidney. The cut renal parenchyma was covered with surgical adhesive in aerosol. The use of this tissue adhesie aimed at elimination of ischaemia developing after suture application. After the operation the kidneys were obtained for examinations after 7 and 14 days and 1, 3 and 6 months. Angiographic, micro-angiographic and histological examinations demonstrated that transverse ampu­tation of the renal pole is an operation sparing better renal parenchyma than wedge resection. In the latter operation early observations demonstrated exten-sive areas of necrosis and impaired blood flow, and after 6 months atrophy and fibrosis of parenchyma was observed. After healing the loss of actiye layer of the kidney was greater after wedge resection. In the light of this experiment wedge resection; of the renal pole was evaluated as a non-anatomical operation in relation to vascular segments, and thus it is less useful than transverse amputation.
