Objectives. The compliance between clinical and urodynamic criteria in qualification to the surgical treatment of BPH were examined.
Material and methods. Sixty patients, aged from 60 to 81 yeras (mean: 69,7 years), qualified to surgical teratment due to BPH, were enrolled in this study. The complexe urological examination, including urodynamic estimation of the lower urinary tract, were perform before, as well as 3-6 months after surgery. Fifty five men underwent transurethral resection of the prostate (TURP), and 5 patients underwent adenomectomy (OPSU). Efficacy of the treatment was done on the base of standard mks turiction characteristics wich were compare before and after surgery. The subgroup of patients in whom surgical treatment was uneffective was carefuly analysed.
Results. In 53 patients TURP and OPSU improved the function lower urinary tract, and 7 patients the teratment was uneffective. Pre- and postoperative urodynamic analysis of these 7 patients revealed, that 3 of the them were nonobstructive with weak [2] or nromal [ 1 ] detrusor function, 2 patients with weak detrusor were in middle zone before operation, and 2 patients had bladder outlet obstruction and normal detrusor before teratment. After operation 6 patients were found in nonobstructive or middle zone, and all of them demonstrated weak detrusor function. In 1 patient urodynamic evaluation showed bladder outlet obstruction and normal detrusor function. Conclusions. In a group of patients with BPH and relative indications to the surgical treatment, an urodynamic cvalution is enable to anticipate probable changes of lower urinary tract function as a result of the opreation. Patients without bladder outlet obstruction and with detrusor weakness have the greatest risk of surgical failure. In this group of patients the choice of alternative treatment appears justified.
