ESWL treatment for staghorn nephrolithiasis was applied in 22 patients, ages 17 to 66. In one patient the calculi were present in both kidneys. ESWL was performed with the use of Lithostar lithotriptor. The results of the procedure were evaluated after 12 months following the first lithotripsy.
Good treatment results (total removal of calculus or presence in the kidney of some fragments of concrements, the longest dimension of which did not excede 4 mm) were obtained in 14 kidneys (47.8%). Fragments of calculi incarcerated in the ureter, causing the necessity of emergency urinary diversion from the kidney, occured in 10 patients (45.4%). Surgery, as the result of ineffectiveness of ESWL or of complications following ESWL, was applied in 3 patients (13.1%).
Indications for treatment of staghorn nephrolithiasis by the ESWL method alone should be limited. ESWL is justified in patients with nephrolithiasis involving only partial filling of the renal pelvis and calyces, where the calculi fill non dilatated pyelocalyceal system. Application of internal drainage of the upper urinary tract with „double J\" catheter is advisable in these patients.
